What to expect from your medical assessments
Patients will have two 1-on-1 appointments with an MD for one of our treated conditions.
1st Appointment: Once we receive a referral from an MD/Nurse Practitioner, we will send you an online questionnaire to complete. Once we receive this completed questionnaire, we will invite you to book an first appointment. At the first appointment, an MD will do a preliminary evaluation and then ask you to keep a specific journal for your condition that you will keep in real-time for 1 week. At your second appointment, an MD will have this journal to help inform treatment decisions.
2nd Appointment: Once we receive the completed journal from your first appointment, you will be invited for a 2nd appointment. During this appointment the MD will review your journal and decide if you are suitable for OHIP-funded Group Counselling Sessions for your specific issue.
