Administrative Support and Disability Forms

Patients often request or authorize a release of their medical information.  To do this, please download and complete the Patient Consent Form and fax/email it back to us.  We will then notify you of the cost of this request as per our uninsured services policies.  Sometimes patients will ask our team to prepare disability reports or reports for workplace accommodations.  Please email us the form you would like to have completed along with a brief explanation and the due date.  We will then notify if we can perform this task for you and the cost of the work.  Sometimes, we cannot meet the deadline or cannot complete the task as it is more suitably done by your GP or specialist (e.g., ODSP applications).  Payment is required before we can perform any task.
