DESCRIPTION OF EMPLOYER

The Toronto Concussion Clinic (torontoconcussion.ca) is a 4-year-old initiative, focused on providing comprehensive, patient-centered care to help concussion patients recover. The first out-patient clinic of its kind offering mostly OHIP-funded neurological rehabilitation care in novel settings with fresh medical paradigms, it provides professionals with a dynamic, versatile and progressive practice environment. It has a vibrant, multi-disciplinary team with cutting-edge approaches and technology. This role will attract the professional looking to be a ‘part of something new’ and apply skills in many areas – history taking, collaboration with allied health workers, patient rehabilitation, patient education, counselling, program development, providing treatments, and the use of informational technology in the dissemination and standardization of best practices medicine, etc.

 

POSITION SUMMARY

Working interdependently with the medical doctors, the RN will help with:

 

Direct Patient Care

  1. Regular observation of patient logs and frequent case management/follow-up
    appointments to ensure effective strategies are used by patients. E.g., appropriate
    energy management, healthy sleep habits, exercise prescription compliance, reintegration
    to work/school and sport, support with psychological endeavours, implementation of
    cognitive remediation/compensation measures, etc. More information regarding the

  2.  Provision of relaxation and mindfulness treatments, under the supervision of the
    medical director
  3. Facilitate group counselling sessions
  4. Activities include and are not limited to: interviewing patients; motivating patients
    to comply with treatment; implementing problem-solving strategies to help patients manage
    their prescribed treatments. Methods include CBT, motivational interviewing, solution-
    focused therapy, S.M.A.R.T. GOALS and other methods of performance enhancement

Communication

  1. Use clear and efficient communication with supervising physicians and the allied health team including the maintenance of the cumulative patient profile (CPP) so that it is comprehensive, efficient and up-to-date
  2. Utilize encouraging and assertive language with patients

 

Medical Documentation and Record-Keeping 

  1. Perform proper documentation and record-keeping to fulfill standards of the clinic
  2. Understand, support and evolve with clinic processes/procedures

 

Collaboration with the Health Care Team 

  1. Avoid patients falling in between the “gap” by serving as a case manager between your supervising physicians, allied health team and office administrators
  2. Facilitate continuity of care for patients accessing services at the clinic

 

Administrative: 

  1. Maintain, and assist in the development where applicable, efficient modes of communication between members of the health team and between the health team and patients
  2. Maintain, and assist in the development where applicable, of efficient modes of documenting patient clinical information and treatment directions so that all members of the team are ‘on the same page’ at all times as well as other physicians involved in the patients’ care
  3. Perform office administrative tasks when required

REPORTS TO 

The supervising physician and/or managing director for clinical and administrative direction, supervision, professional roles and responsibilities

 

EDUCATION AND QUALIFICATIONS

  1. Diploma in a program dealing with brain injury rehabilitation, behavioural psychology
    or similar.ORDiploma in Nursing and registered in good standing with the College of Nurses of Ontario
  2. Mental health experience an asset
  3. Experience with exercise/sports involvement/case management an asset
  4. Proficient computer skills: clinical documentation and communication in EMR with an aptitude to learn new skills in different venues that support clinical initiatives as outlined above
  5. Excellent communication skills, keen problem-solving ability, willingness to learn
  6. This is a program that will be very satisfying for professionals that identify themselves as people persons and have a sense of humour
